Kovarian has now fooled the Doctor twice by using Gangers. She even commented on how amused she was to catch him with the same trick twice.
There'd be a beautiful symmetry if the Doctor's death at Lake Silencio was a reversal of the same trick. Assuming that a Ganger Doctor can (try to) regenerate, that is...
And, yes, Canton made a point of stating that this really was the Doctor and he really was dead. But the Doctor lies - especially when outsmarting his enemies - and he can be a bad influence on his friends, too... :-)

However things I'm not quite sure:
- Why did the young river need to be in a space suit. Indoctrination on earth?
- Why do they need an elaborate astronaut emerges from lake to zap the doctor thing anyway? I mean, the timeline needs the killer to be anonymous beforehand, and River does go to prison for killing 'a good man'. I did think that might be bait and switch, and it was actually Rory who was due to die (because he's not done it recently, after all).
- The Doctor quotes his age when he shows up at the lake. Why? Is it to make clear he's way older, and he's had plenty of time?
- He did have 32 minutes of poison, he 'spent' most of them somehow. 32 minutes + the 'meet at 16:30' on his invitation, means 5:02pm, his time to die.
- What's the question that silence will fall when it's asked? (Hiding in plain site, I'd be thinking it'd be 'Who?' or some variant, but perhaps it's River asking him 'are you ready?'
- the girl in the suit - presumably River. So she knows what's going on when she's going back there, with the Doctor to 'sort things out'.
- Why do we get - mostly - reversed order River? First meeting was when she died, and it's been stepping backwards since - at least, mostly.
- the childs song - tick tock, goes the clock. It's foreshadowing, but _what_ is it foreshadowing? And why/how did it get to the strange dollies, and to be repeated by Kovarian?
- Who is Kovarian?
- What's the agenda behind the silence?
- And why did the Tardis explode, anyway.

Looking at it, it may be a prophecy of sorts, but it doesn't actually imply that the Doctor is going to die. I think we're just supposed to read it that way, because we've been told up front what's supposed to happen.

Why is the dying doctor 200 years older if the doctor we see is about to go to his assigned death?
If the Doctor can be killed by a simple gun, why did Kovarian have to go to the trouble of breeding a half-time-lord assassin? And why does she have to be in a space suit?
